Weform is a mentoring platform focused on formation and meaningful connection for college-age young adults. During the fall, students participate in a cohort with their peers, exploring identity, purpose, and direction before being matched with a mentor. That’s where you come in. As a mentor, referred to as a “former” at Weform, you’ll be paired with one student who has completed that work and is ready for a meaningful one-on-one connection.

Here’s the commitment: This fall, you’ll record a short video and write a note to your student through the Weform app. Both will be delivered on December 2 and will serve as your student’s first introduction to you. In the spring, you’ll meet one-on-one four times for 30 minutes each, on Fridays between 11:00 a.m. and noon Central: January 22, February 5, February 19, and March 5. Conversations will take place by video within the app, so you can join from anywhere. After each conversation, you’ll confirm that it took place so we can accurately track mentorship hours.

You don’t need to have everything figured out, and you’re not expected to solve every problem. These students aren’t looking for a résumé review. They’re looking for someone a few steps further along who will listen, share perspective, and offer honest guidance.

This form takes about four minutes to complete and includes a few short-answer questions. There is no “right” answer, so please respond based on your own experience. The goal is to determine fit: whether the scheduled Fridays work for you, whether you can commit to all four conversations, and what you hope to offer a student. Responses are not ranked. INTAKE FORM

Mentoring Goals: What do you hope to offer your student, and what would you like them to take away from the mentoring experience?

Matching Preferences: Are there particular types of students or topics you would especially like to support? Are there any topics you would prefer not to take on?

Your Experience: What experiences, lessons, or decisions have been most important in shaping your life and career? A few sentences are plenty.
